A collaborative project that utilised clinical audit and review by specialist pharmacists to identify patients with AF who were at a high risk of stroke and who were not prescribed optimal anticoagulation therapy. Practices also carried out quality improvement activities to embed learning and make sustainable improvements to the management of their AF patients.
